The clearance that set you up to fail
You got cleared to return to sport. Your strength is 90% of the uninjured side. You can jog without pain. Your physio says you’re ready. Two weeks back, you re-injure yourself.
This is one of the most common scenarios in sports medicine, and it happens because most return-to-sport clearances are incomplete. They test the wrong things, or they don’t test thoroughly enough.
Why standard clearances fail
- Strength testing is insufficient. A single strength test doesn’t tell you how the limb handles sport-specific forces.
- No sport-specific assessment. You can have great strength in a rehab setting and still fail a sport-specific movement.
- Psychological readiness is ignored. Fear and confidence have a big impact on injury risk.
- Training load isn’t managed. You might be “ready” but the team’s plan puts you at risk. Return is gradual, not a switch.
- No follow-up. You’re cleared, then you’re on your own.
A better clearance
Strength that matters: at least 90% of the uninjured side, and crucially eccentric strength (controlling the lower, the landing, the deceleration), because that’s where re-injuries happen.
Movement quality: single-leg stability without excessive hip drop, hop testing for distance and time, and sport-specific movements done smoothly.
Reactive and fatigued testing: sport isn’t done in isolation. Test movement reactively, under fatigue, and under decision-making stress, because that’s when quality breaks down.
Psychological readiness: does the athlete feel confident, or fearful? Fear of re-injury is a major risk factor, even in a physically “ready” athlete.
The return-to-sport decision
Clear an athlete only if strength is at least 90% (concentric and eccentric), movement quality is symmetrical, sport-specific movements are pain-free and smooth, psychological readiness is present, and training load is planned as a graduated return. If any of these is lacking, they’re not ready.
The graduated return
Return isn’t a single event. It’s a graduated process over 4-6 weeks: controlled non-contact training, then light contact at moderate intensity, then higher intensity with full contact, then progressive return to full training and competition. This assumes the athlete passed all testing. If they didn’t, extend the timeline.
Monitoring after return
Clearance isn’t the end. The first 4 weeks back are high-risk. Monitor pain, swelling, movement quality under fatigue, training load, and confidence. If any are concerning, dial intensity back temporarily.
The bottom line
If your return-to-sport clearance is just a strength test, it isn’t enough. A proper clearance is comprehensive strength testing (concentric and eccentric), movement-quality testing in sport-specific positions, psychological readiness, sport-specific training, a graduated return, and ongoing monitoring. You deserve better than a date on a calendar.
