The injury that just won’t stay fixed
You finish rehab. Your hamstring feels better. You return to running. Three weeks in, you feel a pop. Hamstring strain, again. This pattern is incredibly common: re-injury rates sit around 30-40%. It shouldn’t be that high, but it is, because most hamstring rehab is incomplete.
Why they keep coming back
- Eccentric strength is undertrained. Most rehab focuses on isometric or concentric strength, but strains happen during eccentric loading (lengthening). You need eccentric strength to prevent re-injury.
- Running-specific training is missing. Great strength in a rehab setting doesn’t transfer to the forces and speeds of running.
- Load management on return is poor. Most athletes return to full training too fast, sprinting before they’ve eased back in.
- Flexibility is gained early, then lost as you tighten up again, without ongoing maintenance.
- Single-leg control isn’t tested. Running is a single-leg activity, but most rehab is done on two legs.
The protocol that prevents re-injury
Early rehab (weeks 0-2): reduce pain, restore basic function. Gentle isometric hamstring holds, easy stretching after a warm-up, and walking to promote movement.
Build strength (weeks 2-6): progress to heavier loading, with eccentric work specifically. Nordic hamstring curls are the standard here, alongside single- and double-leg glute bridges, light deadlifts, and hamstring curls. Nordics are hard, but they’re the best eccentric exercise there is.
Running-specific (weeks 4-12): a gradual return to running. Walk-jog intervals first, building toward easy continuous running, then tempo efforts. No increase greater than about 10% per week.
Advanced (weeks 8-16): add speed and power. Strides, tempo runs, hill running, and single-leg plyometrics, with Nordics continuing throughout.
The key difference: ongoing eccentric work
Most athletes finish rehab and stop doing Nordic curls, then forget the very exercise that prevents re-injury. Hamstring prevention requires ongoing eccentric training: Nordic curls, roughly twice a week, throughout your athletic career. It’s the single biggest predictor of staying injury-free.
Testing for return
Before returning to full sport: hamstring strength at least 90% of the other side, pain-free running for around 30 minutes, pain-free running at your sport’s speeds, maintained flexibility, and symmetrical single-leg control.
The bottom line
Hamstrings re-injure so often because eccentric training is incomplete, running progression is poor, prevention work gets abandoned, and return to sport is too aggressive. Fix those four things and the re-injury rate drops sharply. Nordic curls, twice a week, for life.
