Meniscus Tear: Surgery vs Rehab

The study that changed how we think about meniscus surgery

A decade ago, a meniscus tear usually led straight to surgery. Then a landmark trial compared keyhole meniscectomy with supervised physiotherapy, and both groups improved equally. Surgery added nothing. Since then the evidence has been consistent: most meniscus tears respond well to rehab, not the knife.

What your meniscus does

It’s a C-shaped cartilage between the thigh bone and shin bone that spreads load, absorbs shock, and helps stabilise the knee. A tear sounds alarming, but these injuries are extremely common, and plenty of people have them with no symptoms at all.

Surgery is overused

The research shows keyhole meniscectomy performs no better than physio for most tears, especially degenerative ones. Removing meniscus tissue actually raises long-term arthritis risk, recovery is often slower than good rehab, and re-tears happen. Surgery should be the exception, not the default.

When surgery might help

It’s unlikely to help degenerative tears, pain without mechanical symptoms, or cases that haven’t tried rehab yet.

The rehab that works

Weeks 0-2: control swelling, avoid full immobilisation, get moving early with walking, gentle knee bends and quad activation. Weeks 2-6: progressive loading (pain-free squats, step-ups, glute bridges, lunges), because the meniscus responds to appropriate load. Weeks 6-12: single-leg strength for real stability. Weeks 8-16: return to running and sport-specific work.

The timeline

An uncomplicated tear usually takes 8-12 weeks of proper rehab to return to sport. Surgery is a similar timeline overall, but with meniscus tissue lost.

The bottom line

A meniscus tear isn’t automatically a surgical problem. It’s a signal the knee needs attention, and that attention almost always comes through targeted rehab. Try physio first, load the knee progressively, build strength, and return to what matters. Surgery stays available if it’s ever truly needed, but the odds you’ll need it drop sharply with a genuine rehab effort.
